Obviously the hook hole in the trolley load suspension plate is worn. In addition I would suspect some hook seat wear. Cause: I suspect a swinging load or a repeated side load application. Root cause: I would take a closer look at the suspension plate. Lack of paint on the plate, very rough torch cut and no chamfer on the outer edges of the hook hole make me consider that the plate was custom made by the end user and lacks benefit of heat treatment that would improve wear resistance at the hole in addition to assisting in overall strength. If all checks out in this area, perhaps the end user needs to assure true vertical lift in his application by moving the beam or moving the lift point.
